I have always deleted all the files recommended from spybot S&D and
adaware...however, it is not always the best solution for everyone.
There are file that could be recommended for deletion that could well
be harmless and necessary for your software. One example I can think
of is BackWeb, which is used by a number of spyware programs, but is
also used by some legitimate programs too. Your best bet, if you are
not familiar with how to interpret the findings, is to quarantine them
and see how your system performs without them. If everything operates
the way it should, then delete the quarantined files. It's always
better to be safe and BACKUP your files when removing things you don't
    completely understand.

Well, do you also delete files marked as "directory", and "registry
change"(DSO Exploit)?? I use Spybot S&D, but am too afraid to delete
the registry change/directory files - then again I am clueless about
    all this.
